What We Offer
Homeward Connect Visit
Following hospital discharge, our MCLs ensure patients return home safely, assist with a sameday virtual visit to their primary care provider, and coordinate any follow-up needs—helping prevent readmissions and supporting continuity of care.
Reconnect Visit
When a patient hasn’t responded to clinic outreach, our MCLs visit in person to facilitate a telehealth appointment with their provider. They remain available throughout the session to provide assistance and confirm completion.
Touch Point Visit
A brief, relationship-strengthening check-in that reinforces provider messaging and keeps patients connected between appointments. Ideal for ongoing engagement and preventive care reminders.
Rapid Connect Escort
For patients discharged from an Emergency Room, our MCLs provide safe, same-day transport to the clinic and accompany them inside for immediate follow-up—turning reactive moments into proactive care.
Care Drop
A light-touch outreach service for delivering educational materials, event reminders, or incentive items to patient homes. Every drop-off reinforces your clinic’s commitment to consistent, compassionate care.

What We Offer
Provider Messaging Visits
MCLs deliver in-person messages directly to the patient, letting them know their provider has requested follow-up and encouraging them to contact the clinic.
Reconnect Visits
MCLs assist patients with connecting to their primary care provider via a telehealth portal, walking them through the process step-by-step. Once connected, the MCL steps away (but remains on-site) until the visit ends, then documents completion and obtains a patient signature.
Patient Reach Initiative
As part of outreach, MCLs deliver goodie bags (provided by the clinic or through partnership arrangements) and invite patients to local events focused on reconnecting with care teams.
