Personalized, in-home outreach to help value-based clinics reconnect with hard-to-reach patients and close care gaps.
Why This Program Was Created
The Bridging the Gap Outreach Initiative was developed to support value-based healthcare providers in reconnecting with patients who have become disengaged from care. Despite repeated outreach attempts by clinic staff, some patients remain unresponsive to phone calls, mailers, and messages. This leads to open care gaps—services or screenings that were missed or undocumented during previous visits—and ultimately delays critical care while impacting the clinic’s ability to meet quality metrics.

What We Offer
Provider Messaging Visits
MCLs deliver in-person messages directly to the patient, letting them know their provider has requested follow-up and encouraging them to contact the clinic.
Reconnect Visits
MCLs assist patients with connecting to their primary care provider via a telehealth portal, walking them through the process step-by-step. Once connected, the MCL steps away (but remains on-site) until the visit ends, then documents completion and obtains a patient signature.
Patient Reach Initiative
As part of outreach, MCLs deliver goodie bags (provided by the clinic or through partnership arrangements) and invite patients to local events focused on reconnecting with care teams.
